FERPA Forms

Request to Withhold Directory Information – Lafayette College designates certain categories of Student information as public or “Directory Information” and may disclose such information at its discretion. Currently enrolled students may request that the College withhold disclosure of Directory Information by submitting this form.
Request to Release Hold on Disclosure of Directory Information – If a Student has submitted a Request to Withhold Directory Information form, directory information will then be withheld until the Student releases the hold by submitting this form.
Web Proxy Management System for Students – Students may grant anyone access to view elements of their student record that are protected by FERPA using the Web Proxy Management system in Banner Self-Service.
Recommendation Release Form – Students should complete this form before returning it to a Lafayette College faculty or staff member whom you are asking to write or submit a recommendation or refererence.
